The Family Physician’s ACO Blueprint for Success, has been reviewed and is acceptable for up to 2 Prescribed credit(s) by the American Academy of Family Physicians. AAFP accreditation begins April 14, 2011. The term of approval is for 1 year(s) from this date, with option for yearly renewal. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
